Аннотация

Быстро меняющаяся демографическая и социальная ситуация, старение населения и связанное с этим растущее число реально нуждающихся в долгосрочном уходе, но остающихся вне зоны действия системы социальной поддержки, ставят перед государством новые задачи. Модель государственного обеспечения только самых бедных и уязвимых и опора на местное сообщество как организатора социальной сферы и семью — как опору для всех остальных не в полной мере отвечает новым вызовам. Статья посвящена современному пониманию процессов старения и определению долговременного ухода как нового социального риска; анализу различных подходов к формированию систем поддержки лиц с ограничениями в самообслуживании на примере ряда ключевых государств с высокими темпами старения и средним уровнем развития, где предложение неформального семейного ухода сокращается, а инфраструктура только формируется. Пролонгированная и многоканальная система современного ухода относится к категории дорогостоящих услуг. Их стоимость, как правило, значительно превышает медианные доходы пенсионеров в случаях средней и высокой зависимости. Среди дополнительных ограничителей в большинстве среднеразвитых государств выступает и невысокий уровень доступности медицинских услуг. Индикатором последнего является высокая доля личных расходов пациентов в общих затратах на здравоохранение. Практика рассматриваемых стран показывает, что задачи формирования современных институтов, которые могли бы обеспечить повышение эффективности общественных расходов на цели материальной и социальной поддержки пожилого населения, решаются на пути создания смешанной экономики ухода с использованием средств государственных бюджетов, страховых схем и населения, с привлечением государственных, некоммерческих и коммерческих структур, действующих в данной сфере. Формы и объём государственной поддержки в каждом случае определяются размерами опекаемых групп населения.
